AUDIT Forms
The Alcohol Use Disorders Identification Test (AUDIT) is administered to adult clients who screen positive on the pre-screening for alcohol use. The first part contains the screening questions and side two contains instructions on scoring and interpreting the AUDIT. Both a clinician-administered version and a self-report version of the AUDIT are provided below.
